Respect for our clients' privacy is a priority at Chair & Equipment Rentals & Sales, Inc. This privacy note is provided to help you understand how we handle your personal information collected during business transactions. The trust of our customers and potential customers is a valued asset in our business. This trust is built, in part, upon the proper handling of personal information.

Although technology has changed the way information is gathered, used and stored, the importance of preserving the confidentiality of information remains one of our core values and the responsibilities.

We protect all personal information we collect about you by maintaining physical, electronic, and procedural safeguards that meet or exceed the applicable law.

THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION.

We protect personal information we collect by maintaining physical, electronic, and procedural safeguards that meet or exceed the applicable law. We train our staff to properly handle personal information. Access to your information is restricted. Third parties who have access to personal information must agree to appropriate standards of security and confidentiality.

When we bill third party payers such as, Medicare, Medicaid and private insurance, we will collect personal information from you when you submit a prescription for durable medical equipment (wheelchairs, walkers, liftchairs, etc.) This information comes from various sources including:

  • Information we receive from you on claim forms such as, name, address, social security number, telephone number, and occupation.
  • Information about your diagnosis from your doctor.

We do not disclose any personal information about you or our former customers except as permitted by law, such as disclosures necessary to process and service your account and to protect against fraud and to protect the security and confidentiality of your records.

We will safeguard, according to strict standards of security and confidentiality information our customers share with us.

We will limit the collection and use of customer information to a minimum that is required to deliver superior service to our customers, including advertising about our products, services and other opportunities, and to administer our business.

We will permit only authorized employees, who are trained in the proper handling of customer information, to have access to that information. Employees who violate our privacy policy will be subject to disciplinary measures.

We will not reveal customer information to any external organization unless:
  • We have previously informed the customer in disclosures or agreements.
  • We have been authorized by the customer.
  • We are required by law or our regulators.

We will not use or share, internally or externally, personally identifiable medical information for any purpose other than the administration of a customers claim or as disclosed to the customer when the information is collected or to which the customer consents.

Paperwork containing confidential customer information that becomes inactive will be disposed of in accordance with acceptable standards.

We will continually assess ourselves to ensure that customer privacy is maintained. We will conduct business in a manner that fulfills our promise throughout the piedmont region.
